Appointment Policies

Discover Our Unique Healthcare Philosophy

Because of time constraints, please be respectful of your allotted appointment. If for any reason you are not able to make your scheduled appointment, we require 24 hours notification. This will allow us to fill your appointment slot with someone who may need care urgently. Otherwise, you are considered a “no-show” and will be billed $75 for this missed visit. We make every effort to provide courtesy email and test reminders both 2 days prior to your appointment and 2 hours to allow time to cancel without penalty and arrive timely for your appointment.

Unscheduled appointments (STD/Urgent Care) after 8pm are considered after-hours and require a $75 credit card hold. If you cancel after scheduling you will be changed $75.

We realize things do come up and will take this into account individually, but wasted appointment slots hurt other patients as well as the practice. Please understand the need for this policy.

Medical Records

The medical record is the property of Mobile Care LLC and we are required to keep the originals for 7 years after our last visit with you. We will provide copies as needed. Electronic copies may be provided at no charge. Printed copies are charged at $0.18/page printed.

Referrals

If you are required by your health insurance carrier to obtain a referral to be evaluated by a specialist, then Mobile Care and you the patient are both bound by the insurance company contracts to have your Primary Care Provider first evaluate this issue PRIOR to issuing a referral. Providing quality care will sometimes require involving a specialist, trust that your PCP has your health in mind but MUST comply with your insurance policy’s regulations.

COVID-19

This company policy includes the measures we are actively taking to mitigate the spread of coronavirus. You are kindly requested to follow all these rules diligently, to sustain a healthy and safe workplace in this unique environment. It’s important that we all respond responsibly and transparently to these health precautions. We assure you that we will always treat your private health and personal data with high confidentiality and sensitivity.

This coronavirus (COVID-19) company policy is susceptible to changes with the introduction of additional governmental guidelines.
Employee Policy
  • Any employee or provider who becomes ill with COVID-19 symptoms will complete an approved test.
  • All employees and providers have received a vaccination.
  • If any provider or employee at Mobile Care LLC has a positive COVID-19 diagnosis, they will follow current CDC guidelines and not see patients for an estimated 7-10 days.

Work at Your Home or Office

  • Mobile Care LLC provides services at your home or office. We kindly request that you notify us if any person in your home or office is acutely ill in the last week, prior to your visit.
  • Mobile Care LLC will perform a COVID-19 screen on all patients before rendering services and reserves the right to refuse service if a patient and another person in your home or office appear acutely ill and contagious. We will make every effort to move to a more suitable location before the cancellation of an appointment.
  • All of our equipment is cleaned between each use using cleaning products that are effective in killing the COVID-19 virus.

Men

General Hygiene Rules

  • Wash your hands after using the toilet, before eating, and if you cough/sneeze into your hands (follow the 20-second hand-washing rule). You can also use the sanitizers you’ll find around the office.
  • Cough/sneeze into your sleeve, preferably into your elbow. If you use a tissue, discard it properly and clean/sanitize your hands immediately.
  • Open the windows regularly to ensure open ventilation.
  • Avoid touching your face, particularly your eyes, nose, and mouth with your hands to prevent getting infected.

Clayton Evans

Clayton Evans is an IFA Certified Personal Trainer and IFA Certified Sports Nutritionist with over 5 years of experience in the health and fitness Industry. After years of struggling with his own health and fitness, Clayton lost over 30 lbs and started a career helping others achieve the best version of themselves. Clayton spent several years working with large corporate gyms developing fitness programs and working with Nutrition and Wellness Companies to improve a client’s overall health. His approach with clients is simple – small changes in fitness and diet that will have the largest impact and will fit into a client’s busy lifestyle.

Chad Antonson

Chad is a native of Colorado, growing up on the western slope. He moved to the Denver area in 2019 after working as a wild land firefighter through Job Corps. In his free time he enjoys hiking, skiing and traveling.

Brooke Young

Brooke is a skilled and compassionate Adult Nurse Practitioner with over 10 years of experience in healthcare and a passion for helping patients achieve optimal health and wellness. Brooke holds a Master of Science in Nursing with a specialization in adult health and gerontology from Maryville University of St. Louis. She completed post-graduate training in functional medicine through the Institute for Functional Medicine and obtained her NP board certification through the American Academy of Nurse Practitioners Credentialing Board in 2019. As a Nurse Practitioner, Brooke seeks to address the root causes of disease and treat the whole person, rather than just the symptoms. She believes in taking a personalized and holistic approach to each patient. She has special interests in helping patients heal from metabolic syndrome, achieve their weight loss goals, optimize fertility, and recover from complex chronic illness. In her free time, Brooke enjoys hiking, reading, rollerblading, cooking healthy meals, and spending time with her fun-loving husband and spirited toddlers. She is excited to partner with you on your health journey and support you in achieving your goals.

Kyle Christensen

Kyle is a career Firefighter/Paramedic with extensive experience in the Denver metro area. He holds a bachelors of Health and Exercise Science from CSU. Kyle enjoys weightlifting, skiing, boating, and all the outdoor activities Colorado has to offer.

Jacqueline Carbone

Jacqueline Carbone is a highly experienced nurse practitioner that specializes in providing her clients with holistic, evidenced based, compassionate care. She specializes in Mental health, assisted therapy and Migraines. She is board certified from the American Nurses Credentialing Center. Jacque has her post Masters certificate from University of Colorado in Psychiatric Mental Health and two master’s degrees—one in acute care/geriatric nursing and one as a clinical specialist in geriatrics—from Georgetown University. She has also completed a fellowship with Dr Arnaldo DeSilva a headache specialist. She is passionate about providing a holistic treatment plan for her clients.

Steve Ohl

Steve is a native of Colorado with bachelors in culinary nutrition. He worked as a classically trained chef, for over a decade. After chef life he shifted careers to join Mobile Care Health as our Operations Manager. Steve enjoys Crossfit, boating, road trips and camping.

Dr. Michael McGurren

Dr Michael McGurren was born and raised in Denver, Colorado. He graduated from Creighton University Medical School, and was the Chief Resident followed at Barnes-Jewish in St. Louis. He is board certified in Emergency Medicine and provides medical direction, as well as urgent care services for Mobile Care Health. In his free time he can be found sailing around the world, with a captains license and over 2500 nautical miles sailed. On shore he enjoys traveling with wife and pup Kylo in custom campervan.
